This seems to be a reuse of the The Driller 12 vinyl record with the groove pressed/cut in the runout area from the original 12. The original matrix etchings 'SP. 721-A' (on the A side) and 'SP- 721-B 14539 2 (3)' (on the B side) in still there and in the middle of the groove. The edge of the record seems to be hand cut.

The cover is the lower left/ area (viewed from side A) of the same 12.
