CD packaged in jewel case with clear tray. 12 page booklet includes lyrics, credits, and thanks. While there are no official performance credits noted in booklet, Zakk Wylde is known to have done all vocals and all instrumentation except for drums. Phil Ondich is known to be the drummer on the album.
